Coaches Questionnaire for Team Strength

In order to do our utmost to ensure that ALL teams land in a division where they can be as challenged and competitive as possible, we need your help. At the beginning and the end of every season we ask you to fill out one of the questionnaires below to gauge how well your team did in the past season AND to guesstimate whether you believe the team should be moved into a more or less challenging division for the next season. We realize that these sorts of judgements are never easy to make (especially when asked to look forward) as you can never be entirely sure what your roster will look like in the coming season OR how your players skill levels will improve or diminish in the offseason. We can only ask that you give it your best shot to guesstimate and we will in turn do our best to use this imformation to place your teams in a division to provide them with a FUN and competitive season.

Remember, we will place your team with or without this questionnaire. Without this extra information from you we are really working in the dark and it is much harder for us to ensure good placment.

2008-09 Referee/Coach/Parent Handbook:

Pre-Season Forms:

• Parent Pledge: This form (click here) is to be handed out to your parents at the first practice. Ask them to initial and sign and bring back to you prior to the first game of the season. This is an optional form for you to use as a coach but our experience has shown that coaches employing this form have a much easier time working with their parents over the course of the season.
• Coaches Code of Conduct: This form (click here) is to be signed (one copy each for you and your assistant coaches) prior to the start of the season and returned to LHSC via email.

Ongoing Forms:

• Red Card/Injury: This form (click here) is mandatory and is to be filled out and emailed to LHSC whenever either a red card (or double yellow) infraction occurs during the course of a game OR when there is any sort of a significant injury that occurs during the course of a game, a practice or any sort of team activity.
• Referee Evaluation: This form (click here) is optional and is to be filled out if you have some exceptionally good and/or exceptionally poor experiences with any referees at your regular season and/or playoff games.

Field and Roster Forms

• Blank Field Form: Used for drawing up plays or showing positions --- click here.
• Standard Field Form: Used to assist with player placement. It is set for eleven players, but can be adjusted to fit your needs --- click here.
• Field Position and Substitution Plan: Used to list out starting positions and to organize your subsitution timing and placement. There are four versions of this form (see below). The positions are set using Excel so some quick manipulations can make this form work for any line-up formation or size.

• Official Game Roster: Used to list your roster and to be handed to the referee prior to the game. We have included here both a Word version as well as an Excel version of this roster.
